Doctor X Episode 4 Review
Past Slowly Comes Up
The episode begins with a heartbreaking flashback. Remember when Soo-jung rushed inside the fire-lit place to save a boy? And we do see her saving him. She does save the boy, Baraka, and brings him out, but as she attempts CPR, he dies from the fatal burns. She lived with those children and treated them like family. When she couldn’t save Baraka, she endured the entire blame on herself.
We also see Hui-suk being present with her during the tragedy, and it raises questions about his PTSD as well and how he came into the medical field. Did he want to save people, or did financial need push him? This episode doesn’t give all the answers, but it sure does open the door for them.

Dr Boo Vs The Mannerless Hospital
Doctor/Director Boo confronts the hospital chairman because of the former director’s failure of responsibility during Dr Seo’s case. Since Dr Boo had to endure the consequences, he had to confront him now. He stands up straight and says that the hospital should’ve been careful and must take accountability. However, the chairman seems to be more bothered about the management’s reputation than their moral complications.
What makes this episode interesting is that Dr Boo is fighting something without anyone’s support. While some doctors seem to be neutral, none of them seems to be backing him up. In fact, one doctor even stays on call while being drunk. It shows that this hospital’s mishaps are not just because of one person but of the whole work culture that’s toxic. Dr Boo’s attempt seems to be genuine, but let’s not take things too far right away.

Dr Na’s Unsettling Interest in Soo-jung
Soo-jung gets some strong attention from Dr Na Dae-il. He tries to bring expensive bread, asks her for dinner and approaches her despite her not bothering about him. But what is he truly expecting from her?
One scene particularly shows how much Dae-il crosses boundaries. During an operation, when Soo-jung reaches inside a patient’s body to check a wound, Dae-il legit grabs her hand and proves how doctors who should be professional simply cross boundaries during an important surgery. He also says that Soo-jung is something between a god and a monster. What even? Does he admire her skills or hate her for that?

Meanwhile, Tae-gyeong’s apparent interference in Dae-il’s attempts to approach Soo-jung creates another layer of tension. His hard-working nature makes him easy to root for, but his growing emotional investment in her could complicate their relationship.
Mr Kwon’s Case
Mr Kwon is a pharmaceutical representative, and his case becomes one of the episode’s most disturbing ones. Kwon postpones his liver surgery repeatedly, and one day he collapses. Finally, the doctors around him realise they should’ve educated him to quickly finish his surgery. Now we also see his relationship with plastic surgeon Jo Guk-jin and the doctor’s son’s ideology that seems ugly. Even when the boy notices Kwon suffering, all he cares about is whether his evening classes will be affected. Instead of being humane, he acts selfishly.

The military flashback literally messes up more. Dae-il appears to have saved Kwon when he was badly injured years earlier. However, we see Dae-il’s initials inside Kwon’s organs, and it collapses everything. This is not an ordinary surgical complication that can be dismissed as an unfortunate mistake. This seems unethical and highly dangerous.
Did Dae-il do it, or did someone carve his initials for revenge?
Final Thoughts

Episode 4 successfully shows us multiple sides of the hospital. We see Dr Boo trying to make things right, hardworking doctors doing their job, some evil doctors planning unethical moves and humanity crumbling down. Soo-jung’s past seems sad and develops her character sketch. And Dae-il is a character who is definitely disturbing.
With its unresolved questions and increasingly complicated characters, Doctor X: Mafia in White continues to build momentum. The next episode will need to deliver meaningful answers, but for now, the series has left us with many questions.
