Doctor on the Edge Episode 4 Review: Healing Turns Into Survival

Episode 3 showed how Ji-ui is slowly but steadily gaining the community’s trust. He indeed figures out Jang-su’s medical history and his previous heart attack. This proves to him that his worry was for real and advises Jang-su to follow the prescribed medicine. Jang-su starts trusting Ji-ui. Ji-ui and Ha-ri’s feelings also develop pretty well.

What Stood Out?

The biggest standout point is definitely the second half of the episode, as we see a medical emergency that raises tension. A high-pressure rescue puts the viewers a bit shaken as the characters Ji-ui and Ha-ri test their limits. Yes, while doing an emergency medical aid on the swaying boat, they both remove their life jackets.

At the same time, we see Ha-ri and Ji-ui’s relationship progressing. Ha-ri gets the name Doctor Hunter, meaning she always hangs out with doctor guys to date them. But she doesn’t take too much of this into her heart as she knows that’s not her intent. However, the second male lead, Chi-yeon, becomes pretty evident as he’s often trying his best to capture Ha-ri. This can be the reason behind her getting all those vile names.

That said, with the increasingly obvious love triangle, it does strip away the main script. While some viewers enjoy Chi-yeon’s presence and chemistry with Ha-ri, many felt that his scenes were beginning to take attention away from the core relationship. Fortunately, the episode still makes it clear where Ha-ri’s heart lies, as she straight up boldly tells who she truly likes.

Of course, the final cliffhanger stole the spotlight. Ji-ui ending up in the sea left viewers shocked, a death scare right in the fourth episode of a 12-episode-long series.

The Cast Continues To Carry The Drama

If there is one area where the series seems to keep us active, it is the cast. Lee Jae-wook seems to be proving he’s a strong artist as he is able to portray a cold, unlikable Ji-ui in a way that the viewers seem to agree with him. We can see Ji-ui’s frustration with the community people, his anxiety issues, and his brewing affection with Ha-ri. All these different emotions are well executed by Lee Jae-wook.

Shin Ye-eun continues to steal and seal the emotional and happy aura of the drama. Ha-ri’s kindness doesn’t feel forced, thanks to Ye-eun’s natural acting. The way she also portrays Ha-ri’s boldness and warmth keeps her acting skills on top tier.

Hong Min-gi also deserves credit for making Chi-yeon a good second lead. And a handsome one. Even viewers who dislike love triangles will agree that his charm and chemistry make him admirable.

Balancing Healing Drama And Medical Thriller

Earlier episodes leaned heavily into comedy, village life, and personal growth. However, episode 4 kicks off and introduces medical emergencies that feel too intense and real.

The directing deserves praise for maintaining tension throughout these scenes. The stormy waters, confined spaces, and frantic rescue efforts create genuine suspense. And adding tiny details such as the lead removing their life jackets raised the stakes. At the same time, the drama continues to make excellent use of its island. The location remains one of the show’s strongest assets, providing both visual beauty and heavy challenges to the story.

Not Everything Works

Despite its strengths, this episode has its tiny flaws. The biggest concern for a lot of viewers and for me is the love triangle. Though love triangles are a trope that does have its impact, in a medical drama, it absolutely feels unnecessary, given that the medical emergencies are more than enough to keep us hooked.

Alright, since we do have one in this series, the directors need to shorten their love triangle sequences a bit, as it seems to strip away chemistry between the main leads, Ji-ui and Ha-ri. Because Chi-yeon and Ha-ri seem to be having more scenes when we all know Ha-ri is going to end up with Ji-ui.

The episode’s climax also raised some eyebrows. Several viewers questioned the logic during the rescue scene because some decisions, especially not wearing the life jacket back when the engine is switched on. Emotionally, it worked as it raised tension but logically, it felt off.

Final Thoughts

Episode 4 continues to build the drama toward emotional and narrative stakes. The medical scenes seem to be gripping and improving. The couple’s chemistry also seems to be growing despite the second male lead’s intervention ruining it here and there. All three main leads, Lee Jae-wook, Shin Ye-eun and Hong Min-gi, are carrying the show pretty well.

With the cliffhanger towards the end, the viewers are left with lots of questions until the next episode pops up. We can’t really judge how well the series is going to do based on the fourth episode, but so far, it is a decent watch.
